How can I book an appointment?
Doctors:
PLEASE NOTE WE NOW HAVE CHANGED TO A TELEPHONE APPOINTMENT FIRST SYSTEM FOR ALL DOCTORS APPOINTMENTS.
You can book an initial telephone appointment by either calling the practice at (8:30am) or booking an appointment online through one of the online service providers.
The Doctor will then call you in during or close to your telephone appointment time slot and if you need to be seen will arrange a Face to Face Appointment for you.
If you need an urgent appointment you should always call.
Please call early in the morning if you need an “urgent” book on the day appointment . You may be asked what the appointment is for by the receptionist in order to guide you to the most appropriate clinician. Please answer their questions as fully as you can in order to help them book the best suited appointment for you.
Nurses :
Our nursing team are fully operational and seeing patients who require to been seen in the practice, please call the surgery if you need to book an appointment with them.
Booking online
There are a number of different online service providers that allow you to access aspects of your medical record, book appointments and request repeat prescriptions. To register for these services you will first of all need to complete our online access form, you can do that Here.
Once the forms have been submitted we will also need to confirm your identity by seeing two forms of identification, one will need to be a photo I.D in the form of Driving License or Passport, and the other a bank statement or utility bill. You will have to do this in person however we are trialling a system where if a photo of you holding the forms of identification so we can verify you can be sent to us we will accept this as well.
Not all available appointments are visible online, if you cannot find a convenient time or the type of appointment you need, please call the surgery.
In Dacorum, all the GPs have joined forces and the Covid vaccination centre has been set up in Maxted Road, Hemel Hempstead.
Primary care has been entrusted to vaccinate all patients aged 50 and over and we will be doing so by strict order of priority as advised by the JCVI .
We are all running a text/call service using a system called AccuBook. If you have a mobile phone, you will likely receive a message from your GP surgery inviting you to make an appointment for your jab. If you do not have a mobile phone, you will get a phone call instead.
At the same time, NHS England is setting up a central NHS booking system for its sites, including the one in Stevenage. It is therefore possible that some local residents will receive both a text/call from their local GP and a letter from the NHS about the site in Stevenage. It is possible that you might get the letter from the NHS about the Stevenage site first, and this may well be confusing for some. You as the patient will have a personal choice. If you choose to go to Stevenage, you can. But you are not obliged to – you can get your jab in Maxted Road, Hemel Hempstead, once your GP gets in contact.

Answer: Text message, or a phone call for those who don’t have mobile phones.
Question: Do I have to go to the Stevenage vaccination site?
Answer: No, it is your choice, you can wait to be called by your GP to attend the Hemel Hempstead vaccination centre.
Question: Where will we be getting the vaccine?
Answer: One Central Site in Dacorum.

Registering at our practice
If you are resident in our catchment area ( or temporarily staying at an address in our area ), you are welcome to register at Woodhallfarm medicalcentre.

To register with our practice, you will be asked to fill in a registration form, you can do that here, or you can pick up a paper form from the surgery. If you complete the paper forms, once complete you can either drop them back down to us, or email them to Woodhallfarm.medicalcentre@nhs.net. Registration for Online Access
We also recommend that you register for online access to your records at the same time. There are a number of online providers, the most popular being Patient Access. (by completing an additional consent form and having your a photo-ID checked).
Once you are registered for online patient access you can access online appointment bookings, test results, order repeat prescriptions and also view aspects of your medical record.
Named Accountable GP
Once you have registered you will be allocated a named GP, however you are still free to see the GP of your choice at anytime. Ideally we would encourage you to book with the same Dr for the same medical problem in order to better manage your care.
Temporary Residents
Anyone without a local GP needing to be seen in an emergency but not long-term (e.g. while on holiday or visiting relatives) can be seen as a temporary patient for ‘immediate and necessary treatment’.
They will need to fill out a short form for the information to be returned to their own Doctor.
